New rates of September 3, 2024:
$18 per hour for ages 0-24 months
$16 per hour for ages 2-3 years
$15 per hour for ages 3 and up
15% discount is offered for an older sibling when attending simultaneously
DCFS Child Medical Form Please print and have doctor complete
Lead Risk Questionnaire Please print and have doctor sign
Enrollment is $225.00 for one child and $275.00 for two or more children.
All CLC registration forms, medical forms, including lead questionnaire and birth certificate copy must be turned in before your child’s first reservation. Please contact JoAnn at joann@gortoncenter.org if you have questions regarding enrollment.
